Used Equipment Condition Report

To consider used equipment for leasing or financing, we need the following form completed and each item that is used will need to be listed and information about age, condition, values, etc will be required.  We will also require clear photos of the equipment and these can be uploaded now or sent to your Allstate Capital sales person after submission.  If the equipment is titled, then we will need VIN information and front and back of the title faxed (954-725-2746) or attached.  Make sure to list if any equipment is not free of any liens or encumberances.
